Credentialing Package

Graduates from the MPT Program can request that an official credentialing package be sent by the Department of Physical Therapy to the credentialing agency of their choice. This package does not include official transcripts which must be requested through UBC Enrollment Services. Information on how to request official transcripts can be found at:  www.students.ubc.ca/current/transcripts.cfm. Note that requests for transcripts may have an additional cost that is not included in the credentialing package cost.

Basic Credentialing Package:

  • Certified copies of course outlines and topical outlines of courses successfully completed at the Department of Physical Therapy (formerly the School of Rehabilitation Sciences);
  • A verification letter confirming the number of hours in the program, the accuracy of the documentation content, and an explanation of any discrepancies in the documentation.

There is a minimum $500.00 fee payable by cheque to the UBC Department of Physical Therapy based on the above package outline. Credentialing packages requiring further documentation, authorizations, etc. will be assessed on a case-by-case basis and the fee for service will increase as necessary to compensate the Department for the time required to assemble the documents.

Note: Credentialing packages for the Australian Physiotherapy Council requires extensive processing to review qualifications and academic history. Given this complexity, the fee for these packages is $1200.00.

Fees and timelines for package completion will be assessed upon receipt of the form below. Payment is required in order for the package assembly to begin (the Department can only accept cheque or cash payments).
