Master of Physical Therapy – Standard Credentialing Package:
Graduates of the MPT Program can request an official credentialing package from the Department of Physical Therapy by completing the form at the bottom of this page. There is a minimum package fee of $500.00. Credentialing package requests are assessed on a case-by-case basis, and the fee for service may increase as necessary to compensate the Department for more complex requests. Credentialing requests are reviewed in order of receipt, and every effort is made to process applications as quickly as possible. Please note that on average, applications take four weeks to complete, with more complex applications taking an average of six weeks.
A standard MPT program credentialing package includes:
- Certified copies of course and topical outlines (course syllabi) of courses successfully completed toward the Master of Physical Therapy
- A verification letter confirming the program name, requestor’s graduation date, number of hours in the program, and the accuracy of provided documentation
Please note that the Department of Physical Therapy cannot provide official transcripts. Transcripts must be requested through UBC Enrollment Services: https://students.ubc.ca/enrolment/records/ordering-transcript.
Note: Credentialing packages for the Australian Physiotherapy Council require extensive processing to review qualifications and academic history. Given this complexity, the fee for these packages is $1200.00.
BSc, PT – Credentialing Package:
Graduates of the Bachelor of Science, Physical Therapy delivered by the former School of Rehabilitation Sciences can request an official credentialing package from the Department of Physical Therapy by completing the form at the bottom of this page. Please note that, depending on the year of graduation, Departmental records may be incomplete and packages may not meet the requirements of international credentialing agencies. For information regarding record availability, please complete the attached request form, and email to mpt.admin@ubc.ca. A staff member of the Department of Physical Therapy will be in touch to provide additional details.
