Clinical Site Profiles – Submit your site’s information

Site Profile submission form

Please fill out this online form with details of your site. It will be used to create your site's profile on the Clinical Education section of the UBC PT website. These profiles will be a great help to our students as they search/plan for their placements.

Please note that there is an option to "Save and continue later" on this form. The link is located at the bottom of the form, beside the "Submit" button. If, for any reason, you need/want to save your form and return to finish filling it out at a later time, click on that link. You will be given a link which you can use to directly access your form at a later date (note: link expires after 30 days).

You should also note that this form does not automatically save, so if you are putting in larger blocks of text (e.g., the Welcome Message) we advise that you first type that text in a Word document, then copy and paste into the field. That way it ensures no text is loss in the case of a glitch!

  • This information is simply to place the site (whether public or private practice) in an area of the province dictated by the Ministry of Health regions. If you do not know what region your site is located in please refer to this map.
    “Northern” refers to:
    • Any city or town within the Northern Health Authority Region.

    "Rural" refers to
    • Vancouver Coastal Health: Communities north of Whistler as well as the Central Coast (e.g., Bella Coola), Sechelt and Powell River.
    • Fraser Health: Hope.
    • Northern Health: All locations except Prince George (however Prince George is considered “Northern,” as above)
    • Vancouver Island Health Authority: All locations north of Campbell River, as well as Port Alberni.
    • Interior Health: All locations except Vernon, Kelowna, Penticton, and Kamloops.
    • The Yukon is also considered Rural.
