Clinical Site Profiles – Submit your site’s information

Site Profile submission form

Please fill out this online form with details of your site. It will be used to create your site's profile on the Clinical Education section of the UBC PT website. These profiles will be a great help to our students as they search/plan for their placements.

Please note that there is an option to "Save and continue later" on this form. The link is located at the bottom of the form, beside the "Submit" button. If, for any reason, you need/want to save your form and return to finish filling it out at a later time, click on that link. You will be given a link which you can use to directly access your form at a later date (note: link expires after 30 days).

You should also note that this form does not automatically save, so if you are putting in larger blocks of text (e.g., the Welcome Message) we advise that you first type that text in a Word document, then copy and paste into the field. That way it ensures no text is loss in the case of a glitch!

  • Please provide information about types of learning opportunities that may be available at this site.
  • Please provide information about possible interprofessional opportunities at this site, such working with other health professionals or health profession students.
  • In addition to providing information about the site itself, we like to highlight each site with a picture of the site (and the city/town) and provide basic information on the city/town's population, location, and links to the local tourism site.

We also ask each site to include a "Welcome Message." This message should highlight the personality of the site, offer more information about networking and potential employment opportunities, or anything else you would like the students to know about the site!

You are also encouraged to use this message to talk a bit about the community and give the student a better idea of life in your city/town. The more the students know, the more comfortable they will feel about going on placement in a community they are not familiar with!
  • Max. file size: 20 MB.
We like to include a picture of each site in the search results. If one doesn't exist please take one for us! Landscape orientation preferred. The picture can be of the outside of the building (in the example of a clinic) or of a treatment area, gym, etc. Your choice!

(Note: We have pictures of each city/town - we just need a site picture from you!)
  • Please provide the name of the person who has filled out this information on behalf of the site.
